Clinical Pharmacy is a branch of pharmacy that focuses on the safe, effective, and rational use of medicines to improve patient care outcomes. It involves the direct involvement of pharmacists in patient treatment by working closely with doctors and other healthcare professionals. The main aim of clinical pharmacy is to ensure that each patient receives the most appropriate medication at the correct dose and duration, while minimizing side effects and preventing drug-related problems. Clinical pharmacists monitor drug therapy, identify and resolve medication errors, provide drug information, and counsel patients to improve adherence to treatment. They also play an important role in detecting adverse drug reactions and ensuring medication safety in hospital and community settings.
